We are asking you to be in a research study.  This form gives you information about the research. Whether or not you take part is up to you. You can choose not to take part. You can agree to take part and later change your mind. Your decision will not be held against you.Please ask questions about anything that is unclear to you and take your time to make your choice.
1.   Why is this research study being done?

This is a pilot study, which is a small study that is designed to test out research procedures before they are used in a larger study. Specifically, for this study, we are pilot testing a questionnaire that participants will complete using a smartphone outside of the laboratory during daily life. The smartphone questionnaire will be used in a larger study that is will examine how smokers respond differently to various things when cigarettes are available versus when they are not available.

2.   What will happen in this research study?

After you have agreed to this consent, you will be asked to take a brief survey to determine your eligibility. If you are eligible, you will then be given a more elaborated description on the study and instructions on how to use your phone to complete the given surveys. You will then be directed to a website to register with an application called SurveySignal, which will be used to send you text messages with a link to a survey to complete on your smartphone over a two-day period. You will be asked to enter your name, phone number, and email address when registering. This information will be kept separate from all other information that is collected during the study. After you register, you will be sent a text with a link to confirm your phone number. Once you confirm your number, you will then be sent text messages with a link to complete a survey on your phone at random times throughout the day for two days in a row, starting with the day after you register with SurveySignal. You will be asked to complete 5 surveys each day for the two-day period, and each survey is expected to take 2-4 minutes for you to complete. It is important to note that you should not respond to these texts or attempt to complete the survey in any situations that would be unsafe (such as when you are driving)
